Reasons for undergoing breast augmentation surgery

The choice to undergo breast augmentation surgery is a highly personal one and the reasons behind the choice are varied. Often it is to enhance the body contour of a woman who, for personal reasons, feels her breast size is too small, to correct a reduction in breast volume after pregnancy, to balance a difference in breast size, and as a reconstructive technique following breast surgery.

How is breast augmentation surgery performed?

The method of inserting and positioning your implant will depend on your anatomy and Dr Nerina Wilkinson’s recommendation. The incision will be made in the crease where the breast meets the chest. Every effort will be made to ensure that the incision is placed so that the resulting scars will be as inconspicuous as possible.

Working through the incision, Dr Nerina Wilkinson will lift your breast tissue and skin to create a space for the implant, either directly behind the breast tissue or underneath your chest wall muscle (the pectoral muscle). The implants are then centred beneath your nipples.

Some surgeons believe that putting the implants behind your chest muscle may reduce the potential for capsular contracture. This placement may also interfere less with breast examination by mammogram than if the implant is placed directly behind the breast tissue. Placement behind the muscle, however, may be more painful for a few days after surgery than placement directly under the breast tissue. You’ll want to discuss the pros and cons of these alternatives with Dr Nerina Wilkinson before surgery to make sure you fully understand the implications of the procedure she recommends for you.

The surgery usually takes two hours to complete. Dissolving stitches are used to close the incisions, which may also be taped for greater support. Drainage tubes will be used for several days following surgery. An Elastoplast bandage will be applied over your breasts to help with healing which will be removed when the drains are removed.

Although breast implants don’t actually expire, they aren’t guaranteed to last a lifetime. The average implant can last anywhere from 10 to 20 years at which point it will need to be removed and replaced with a new implant.

A breast implant is a silicone shell filled with either silicone gel or a salt-water solution known as saline. Dr Nerina Wilkinson only uses a silicone-filled implant. Breast Augmentation Surgery: Before and After

In your initial consultation, Dr Nerina Wilkinson will evaluate your health and explain which surgical techniques are most appropriate for you, based on the condition of your breasts and skin tone. If your breasts are sagging, she may also recommend a breast lift. Be sure to discuss your expectations frankly with her. She should be equally frank with you, describing your alternatives and the risks and limitations of each. Be sure to tell her if you have any medical illnesses which may affect your outcome, smoke and if you’re taking any medications, vitamins, or other drugs. When you make the decision to proceed with surgery you will receive comprehensive documentation from her assistant to help prepare you for your procedure both pre-operatively and post-operatively.

Before & on the day of breast augmentation surgery

  • Schedule and complete all pre-operative tests requested by Dr. Wilkinson.
  • STOP SMOKING (6 weeks before and 4 weeks after surgery) – nicotine reduces blood flow to the skin and causes delays in wound healing.
  • Stop taking aspirin, anti-inflammatories, herbal medicine, diet pills, etc. (2 weeks before surgery).
  • Ensure that you have arranged everything you will need for your recovery e.g. meals, pillows, straws, ice, books etc.
  • Report any signs of a cold or infection occurring within 2 weeks prior to your surgery.
  • Arrange for someone to drop you off at the clinic and collect you after discharge. You may not drive for 24 hours after receiving sedation.
  • Schedule a pre-operative consultation with Dr Wilkinson 2 weeks before surgery.

After breast augmentation surgery

After your procedure, you will be transferred to the TCIS overnight facility on the 2nd floor when you have sufficiently recovered from the procedure. Dr Wilkinson will see you at the end of the day to check on your progress and you will be required to stay overnight in the care of our caring and well-trained staff.

The next day Dr Nerina Wilkinson will check on you again before you are discharged from the 2nd-floor facility. The staff will attend to all your needs including pain control, fluid intake and taking you to the bathroom.

What to bring to the overnight facility?

  • MEDICATION – please bring all your chronic medication as well as post-op medication as prescribed by Dr Wilkinson.
  • Do NOT bring valuables or jewellery with you. You will not need them and you wouldn’t want to risk losing them.
  • You may bring your mobile phone which can be locked in the cupboard at your bedside. Please make sure you have your charger and adapter with you.
  • Ensure you have arranged for someone to collect you from the clinic as you will not be able to drive yourself.

Breast augmentation surgery recovery and post-operative care

You can expect some swelling and bruising after surgery which differs from patient to patient. It depends on the extent of the surgery as well as the individual’s tendency to bruise.

  • NO smoking for 4 weeks after surgery. Smoking may compromise wound healing.
  • Do not drive for 2 weeks after surgery

You will be restricted from normal daily activities for 10 – 14 days after surgery but at 3 weeks you should be able to slowly start getting back to normal and at 6 weeks you should be able to do everything you could do before surgery.

You will be required to see Dr Nerina Wilkinson post-operatively after ±2-4 days to have the drains and dressings removed, after one week for a dressing change, after 2 weeks to have sutures removed and at 6 weeks for a surgical follow-up.

Failure to adhere to these instructions may lead to complications and jeopardize the results of your surgery.

What does breast augmentation surgery cost in South Africa?

The costs of the procedure will all depend on the prosthesis we decide to use. (I.e. anatomic or round). This you and Dr Wilkinson will discuss during your first assessment. 

The average cost for breast augmentation is +/- R 85 000 – R 95 000. Some patients may require a lift of the breasts and the insertion of an implant. This can also only be determined at your first assessment.

The average costs of a breast lift/augmentation are +/- R 115 000 – R 130 000. For this procedure, we would require you to stay for 1 night in our overnight clinic.

If you are requiring an extremely natural augmented result then structural fat grafting should be considered.

Is breast augmentation surgery safe?

Breast augmentation is a relatively straightforward procedure and is safe when done with an experienced and board-certified cosmetic surgeon. But, as with any operation, there are risks associated with surgery and specific complications associated with this procedure, including bleeding, infection, or capsular contracture, which will be discussed with you in-depth during your initial consultation with Dr Nerina Wilkinson.

What are the possible risks and complications associated with breast augmentation?

Breast augmentation is relatively straightforward. But, as with any operation, there are risks associated with surgery and specific complications associated with this procedure, including bleeding, infection, or capsular contracture.

Breast implant surgery side effects & complications

Some patients report that their nipples become oversensitive, undersensitive, or even numb. You may also notice small patches of numbness near your incisions. These symptoms usually disappear within time, but may be permanent in some patients. There is no evidence that breast implants will affect fertility, pregnancy, or your ability to nurse. If, however, you have nursed a baby within the year before augmentation, you may produce milk for a few days after surgery. This may cause some discomfort, but can be treated with medication prescribed by your doctor.

Occasionally, breast implants may break or leak. Rupture can occur because of injury or even from the normal compression and movement of your breast and implant, causing the man-made shell to leak. This is a rare complication however as the type of implant that Dr Wilkinson uses has a double shell and the silicone is cohesive meaning that even if the shell breaks the silicone maintains its form without leaking into the surrounding tissues. If a break occurs in the implant, there may be a change in the shape or firmness of the breast which may require a second operation and replacement of the leaking implant.

A few women with breast implants have reported symptoms like diseases of the immune system, such as scleroderma and other arthritis-like conditions. These symptoms may include joint pain or swelling, fever, fatigue, or breast pain. Research has found no clear link between silicone breast implants and the symptoms of what doctors refer to as “connective-tissue disorders,” but the FDA has requested further study. While there is no evidence that breast implants can cause breast cancer, they may change the way mammography is done to detect
cancer.

When you request a routine mammogram, be sure to go to a radiology centre where technicians are experienced in the special techniques required to get a reliable x-ray of a breast with an implant. Additional views will be required. Ultrasound examinations may be of benefit in some women with implants to detect breast lumps or to evaluate the implant. While many women do not experience these complications, you should discuss each of them with Dr Wilkinson to make sure you understand the risks and consequences of breast augmentation.

Where is breast implant surgery performed in Cape Town?

Your breast augmentation surgery will be performed at The Cosmetic Surgery Institute (TCSI) and you will be required to stay at the overnight facility for the first night.

What type of anaesthesia is used when performing breast augmentation surgery?

Anaesthesia for this procedure will be carefully and expertly tailored to each patient. The depth of the anaesthetic will vary from moderate or deep sedation to a general anaesthetic only where necessary. We recognize that each patient’s individual requirements may vary greatly. Most importantly, always, your anaesthetic and sedation will be delivered by a specialist anesthesiologist who will keep you safely monitored and comfortable throughout your procedure.

The Cosmetic Surgery Institute (TCSI) Overnight Facility

At The Cosmetic Surgery Institute (TCSI), your recovery is taken just as seriously as the procedure you choose to undergo. This is why we strive to provide each of our patients with an incredibly comfortable environment where safety, privacy and recovery are of utmost importance. Our overnight facility allows you to experience a relaxing space where you can rest and recover from your surgery. As a Plastic Surgeon, Dr Wilkinson entrusts her patients into the safe hands of the specialised staff at the overnight facility who are trained in the recovery of plastic surgery patients. The facility is also fully licensed and accredited; therefore, you are guaranteed the benefits of a medically controlled environment.

The majority of her patients stay overnight for a minimum of 1 night. However, many patients request to spend additional nights in the facility after experiencing the special care our nurses provide.

Often patients initially request to be discharged on the day of surgery due to their responsibility toward their families. We however feel that a patient’s recovery from surgery can actually cause more stress to their spouse who is already often taking responsibility for the care of the household while the patient is recovering. This is why we recommend spending more than one night in our care where we can take care of the patient and their post-op requirements. You may not want to believe this, but your family can cope without you for a night or two.

The

Procedure

It’s important for you to be honest with Dr Nerina Wilkinson during your first consultation as far as your medical history, chronic medications and expectations of the procedure are concerned.

She will do a physical examination, take measurements, and if necessary photographs to assess your breasts for surgery. She will take into account the size and shape of your breasts, skin quality, and the placement of the nipple and areola. In addition, she will need to be told of any plans (if applicable) to lose a significant amount of weight in the future. Many women find their breasts sag or shrink with weight loss. In this instance, she may recommend that you schedule surgery for once your target weight has been achieved.

Using breast sizers, we will make a decision on the correct size and shape implant for your body. Depending on your breasts and skin tone, Dr Nerina Wilkinson may also recommend a breast lift in conjunction with an augmentation. Be sure to discuss your expectations frankly so that both of you are on exactly the same page.

What Types of Implants Can You Choose From?

There are many incredible choices and Dr Nerina Wilkinson is able to advise on the correct “designer implant” shape that will fit perfectly with your specific breast and chest wall shape.

The choices range from silicone to saline and from rough to smooth textures. Round and anatomically (teardrop) shaped implants are also an option. She has experience in inserting all the type of implants and will take your body type into account before assisting you in the final implant decision.

Is the Implant Placed Under or Above the Muscle?

Dr Nerina Wilkinson’s aim is to give patients the most natural result. Small-breasted women who are very thin are usually advised to have the implant inserted under the muscle to cover the implant. In her view, the advantage of a prepectoral implant (in front of the muscle) is to give the breast tissue a mild lift and is, therefore, the procedure of choice in patients with more sagging breasts. Medical evidence does, however, show that by placing the implants behind the muscle, the potential for the development of capsular contracture is reduced. It is also easier to perform a breast examination and mammography in patients with implants placed behind the muscle. The disadvantage of lifting the chest wall muscle, however, is a longer recovery period. It is therefore of extreme importance that you discuss these pros and cons with her before the procedure.

Once a shape and size has been decided, you’ll book your surgery date.

The procedure will be performed at the Surgical Clinic. Depending on your procedure, you’ll either be admitted as a day patient an inpatient, in which case you’ll be monitored at the Renaissance facilities.

The surgery lasts approximately 2 hours and is performed while you’re under conscious sedation. which will make you feel drowsy and relaxed, however, you’ll sleep through the entire operation and not be aware of the procedure. Occasionally, Dr Nerina Wilkinson may use general anaesthesia, but in all cases, a certified anaesthetist will be responsible for your well being.

The breast augmentation procedure involves:

  • Incisions – The most common site for scars is in the crease under the breast. Other, less common incisions are in the armpit and around the areola. Scars will vary depending on the severity of the natural droop and if any breast tissue needs to be removed. The look of the scars will improve over the course of two years following surgery. As far as the placement of the nipple is concerned, It is usually not necessary that the nipple be transposed higher on the breast. However, in patients where there is sagging of the nipple, a simultaneous breast lift ensures a natural look, while the sensation and functions of the nipple are maintained.
  • Insertion – The placement of the implant will depend on your anatomy and our preoperative discussion. Dr Wilkinson will create a pocket by lifting the breast tissue, either in front or behind the pectoral muscle, and insert the desired implant.
  • Finishing touches – Once the implants are in position, she closes the incisions with sutures and reinforced with tape and a gauze dressing to help with healing. The area is then cleaned and you’re placed in the care of our recovery team. Drainage tubes may also be used for several days following surgery.

Once the breast augmentation surgery has been completed, you can expect to be in some pain for up to 3 days, but you’re encouraged to get out of bed for short periods of time after surgery. It’s also imperative that for two weeks post-op, you don’t raise your arms above your waist, drive or partake in any strenuous activities. If you’ve had drains inserted after your procedure, they’ll be removed in a few days and your dressings will be checked and changed or removed entirely. You’re also advised to handle the breasts gently for the following weeks of your recovery.

To keep pressure off of your breasts, you should sleep on your back. It’s also advisable for you to wear a support bra for a few weeks post-surgery until all swelling and bruising has subsided.

You can expect your stitches to be removed on the 10th day of your recovery and should not be alarmed if your breasts take some time to assume a natural shape and for scarring to fade.

Complete recovery can take anywhere up to 12 months. If you are at all concerned about anything during the post-op period, always contact us for advice and support.

In most cases, it is possible for patients to resume work within a week of breast augmentation surgery unless the nature of the work is excessively strenuous. You can also resume exercise, starting slowly, within a few weeks.

It’s normal for you to experience mild discomfort periodically but severe pain must be reported immediately. Sexual activity may also be resumed after around 2 weeks.
