What is depigmentation?
Depigmentation is a specialised treatment focused on reducing skin pigmentation by targeting the production of melanin. The primary cause of skin pigmentation is an overproduction of melanin, which leads to the formation of dark patches on the skin. Depigmentation treatments effectively work to lighten these dark patches, restoring a more even and radiant complexion.
At Dr. Nerina Wilkinson + Associates, we employ state-of-the-art depigmentation treatments to offer solutions tailored to your skin’s specific needs. By intervening in the melanin production process, these treatments not only lighten existing dark patches but also prevent the formation of new ones. With a comprehensive understanding of the complexities of skin pigmentation, our team ensures that each treatment is carefully curated to produce the most effective and lasting results.
Who is the ideal candidate for Cosmelan/Dermamelan treatments?
Cosmelan/Dermamelan treatments are designed for individuals experiencing uneven skin pigmentation, whether from sun damage, hormonal changes, or other factors. If dark patches, discolouration, or melasma are a concern for you, this might be an ideal solution.
Always start with a professional consultation with qualified and experienced specialists for an in-depth analysis of your pigmentation and to reach a good understanding of your skincare concerns and goals.

How does a Cosmelan and Dermamelan treatment work?
The Cosmelan and Dermamelan treatment begins with an in-clinic depigmenting mask application, followed by an intensive home care regime involving the application of depigmenting creams at home for at least 6 – 9 months following the in-clinic treatment.
We usually recommend coming in for your treatment first thing in the morning.
Part one of your treatment will be performed in the Skin Clinic. The skin will be cleansed and wiped with a degreasing solution. The brown Cosmelan or Dermamelan mask will be applied to the skin in a thick layer.
You will then go home with the mask which needs to stay on for the next 8 to 10 hours.
You will then wash the mask off at home and apply a specific aftercare creams that form part of the Cosmelan/Dermamelan maintenance regime.
Once instructed you begin with part two, the Cosmelan or Dermamelan maintenance cream, you will be advised on the frequency and guided as you start seeing results.
What type of pigmentation can be treated with Cosmelan and Dermamelan treatments?
The most common type of pigmentation treated with Cosmelan and Dermamelan is melasma. Melasma is usually triggered by hormonal changes within the body and will show up as an even pattern often covering the cheeks, temples, forehead and top lip. Sunspots can also be treated effectively as well as post-inflammatory hyperpigmentation, usually seen after acne breakouts.
What areas can be treated with Cosmelan/Dermamelan treatments?
Cosmelan/Dermamelan treatments are medical depigmenting methods designed to take corrective action against dark spots and skin pigmentation issues. Whether you’re aiming to alleviate sun damage or other skin discolourations, or struggle with melasma, Cosmelan/Dermamelan offers an effective solution to bring back the skin’s natural radiance and glow.
What are the benefits of Cosmelan/Dermamelan treatments?
Cosmelan/Dermamelan treatments stand out for their intensive depigmenting action, ensuring a clearer and more harmonised complexion. After undergoing the treatment, patients often witness remarkable improvements in the overall tone and vibrancy of their skin.
The benefits of Cosmelan/Dermamelan include:
- Effectively removes dark spots and reduces skin pigmentation issues.
- Intensive depigmenting action that targets even deep-set discolorations.
- Evens out the skin tone, offering a brighter and rejuvenated appearance.
- Addresses sun damage and other external factors leading to skin darkening.
- Boosts the skin’s natural radiance by eliminating imperfections.
How long does it take to see results from Cosmelan/Dermamelan treatments?
Dark spots and pigmentation irregularities typically start to fade within the first week following the treatment. The full benefits of the Cosmelan/Dermamelan treatments, with a more even and brightened skin tone, become increasingly evident over the subsequent 4 to 6 weeks. Continued improvements can be observed for several months as the skin undergoes its natural regeneration process.
How many Cosmelan/Dermamelan treatments do you need to see results?
For dark spots and superficial pigmentation, most individuals observe noticeable improvement after just one Cosmelan/Dermamelan treatment. However, for deeper pigmentation or more extensive skin discolouration, 2 – 3 treatments might be necessary for optimal results. It is important to note that Cosmelan/Dermamelan in-clinic treatments must be followed by the specific maintenance at-home protocol in order to support and maintain results.
How often should I have a Cosmelan or Dermamelan treatment?
The application is usually a once-off treatment but may be repeated 4 – 6 weeks later if necessary. It is vital to continue with the recommended homecare for at least 6 months following the treatment; this is known as the stabilisation phase.
Some forms of pigmentation will need to be followed by Cosmelan or Dermamelan homecare indefinitely to prevent pigmentation from resurfacing; your Skincare Specialist will guide you through the entire process.
How long does it take to perform a Cosmelan/Dermamelan treatment?
The duration of a Cosmelan/Dermamelan treatment can vary based on the extent of the area being addressed, but typically, the procedure ranges from 30 minutes to 90 minutes. This includes the application of the depigmenting mask and any necessary preparatory steps.
What is the recovery time for a Cosmelan or Dermamelan depigmentation treatment?
Usually, the skin appears red and feels sunburnt for the first 2 – 3 days.
Flaking and peeling usually begins around day 3 and can proceed for up to one week post-treatment.
Once flaking subsides you will already start to notice improvement of the pigmentation. For further improvement and maintenance, you will begin with the home treatment after Cosmelan or Dermamelan. Initially, when using your maintenance creams, the skin will feel sensitive, tight and may flake further, this usually subsides after about 2 weeks.
How much does a Cosmelan/Dermamelan treatment cost?
The cost of a Cosmelan/Dermamelan treatment will vary depending on the extent of pigmentation and the specific needs of the patient. This will be determined during your initial consultation with our specialist.
The average cost for a standard Cosmelan/Dermamelan treatment is +/- R 15 000 – R 23 000. For those with deeper pigmentation or requiring additional sessions, costs may vary.
Additional costs you might encounter include:
Consultation fee of R500
Post-treatment skincare products +/- R 800 – R 1 200
(Please note that these fees are merely estimative and may be subject to change after your consultation with our specialist.)

Are Cosmelan/Dermamelan treatments safe?
Yes, when administered by a qualified Skincare Specialist in a sterile medical setting, Cosmelan/Dermamelan treatments are considered safe. They are designed to cater to various skin types, ensuring effective results across different types of skin. Proper assessment and guidance further ensure the treatment’s suitability for the patient’s specific needs.
What are the possible risks, side effects and complications associated with Cosmelan/Dermamelan treatments?
When a Cosmelan/Dermamelan treatment is administered by a qualified Skincare Specialist in a sterile medical environment, the associated risks are notably minimised.
Cosmelan/Dermamelan treatments, like any other advanced skin treatment, can lead to certain reactions in sensitive individuals. Some may experience mild erythema or redness, which usually subsides within a few hours. On occasion, individuals might notice transient hypo- or hyperpigmentation, indicating temporary lightening or darkening of the skin. There have been rare instances where patients report minor skin irritation, which generally resolves swiftly with appropriate aftercare.
While the vast majority of patients don’t experience these side effects, it remains essential to be informed about these possibilities. It’s always advised to have a detailed discussion with your Skincare Specialist to understand the treatment comprehensively and ensure you’re making the best choice for your skin’s well-being.
Cosmelan/Dermamelan treatments: Before and After
Every client’s skincare journey with us is crafted with precision and individual attention. Our approach initiates with a comprehensive VISIA Skin Care Analysis that provides an objective assessment of your skin’s condition. This is complemented by an in-depth consultation with our Skincare Specialist to understand and address your unique skin concerns.
What to do before the Cosmelan/Dermamelan treatments
- Refrain from using aggressive at-home facial treatments or exfoliating procedures for a week leading up to your session.
- Limit direct sun exposure and abstain from tanning sessions for at least a week before your treatment.
- Familiarise yourself with the skincare products you regularly use, and consider bringing a list with you. This assists your Skincare Specialist in recommending the most appropriate products tailored to your needs.
- Ensure you inform your Skincare Specialist about any medications, past skin conditions, and current health conditions. It’s imperative to understand how over-the-counter or prescribed medications might interact with the treatment.
- If you’re preparing your skin for a significant event and haven’t previously undergone skincare treatments, consider booking your session at least six weeks ahead. Scheduling a follow-up closer to the event date ensures your skin appears its best and any unforeseen reactions are managed promptly.
- Openly discuss your skin goals and concerns. Our Skincare Specialists are trained professionals, adept at creating a comprehensive plan designed around your specific objectives.
- It’s recommended to avoid any aesthetic procedures (like Botulinum Toxin, Facial Fillers, or Threads) for 2 weeks before and after the Cosmelan/Dermamelan treatment.
Day of Cosmelan/Dermamelan treatments
On your first visit to our Skin Clinic, a concise medical questionnaire awaits your completion. This ensures your Skincare Specialist possesses the essential information to select the optimal treatment protocol tailored for you. For the treatment, access to your face, neck, and possibly shoulders will be necessary, so be prepared to change into a provided robe. Your treatment process will entail:
- A thorough skin cleansing
- Application of the Cosmelan/Dermamelan treatment mask
- A resting phase, allowing the mask to penetrate and work on the skin
- Gentle removal of the mask followed by a soothing serum or moisturiser.
After Cosmelan/Dermamelan treatments
Post-treatment, it’s not uncommon to notice some skin redness, akin to mild sunburn, and slight skin tightness. The treated area might undergo a peeling phase as the skin rejuvenates and the pigmented lesions lighten. Proper moisturisation and adherence to the provided aftercare routine will assist in speeding up the recovery. On average, complete recovery and visible results are usually noticeable within a week, with optimal results becoming evident over subsequent weeks as the skin continues its renewal process.
AFTERCARE PRODUCTS AND ADVICE
- Begin and end your day by cleansing your skin using an antibacterial wash with lukewarm to cool water.
- Cooling gels recommended for post-treatment applications can be applied to soothe any irritation. Your Skincare Specialist will offer guidance on the best product choices tailored for your skin’s needs.
- Regularly moisturise with a product advised by your Skincare Specialist, ensuring it supports the healing process and enhances the treatment outcomes.
- Ensure you apply an SPF 50 daily, as your skin will be more sensitive to UV exposure.
- Please refrain from facial waxing for at least 7 days after the treatment.
- Limit direct sun exposure for the initial 2 – 7 days post-treatment, to allow your skin ample recovery time.
- Strenuous exercises or activities that induce excessive sweating should be avoided for the first 24 hours after the treatment.
